Is mercury in amalgam fillings actually a health risk?

What was studied
Regulatory safety reviews (including the U.S. FDA) and epidemiological research examining whether mercury vapor released from amalgam ("silver") fillings is associated with adverse health effects in the general population and in higher-risk groups like pregnant women and young children.
Main findings
Major regulatory reviews, including the FDA's most recent update, report insufficient evidence linking mercury exposure from amalgam fillings to adverse health effects in the general population — including pregnant women and young children — at the low levels typically released (estimated around 0.4-4.4 micrograms per day, well below international exposure limits). A 2014 Cochrane review similarly found insufficient evidence to support or refute adverse effects from amalgam compared with composite. At the same time, amalgam does continuously release small amounts of mercury vapor, which is why some researchers and holistic/biological dentistry practitioners take a precautionary approach favoring mercury-free alternatives, particularly for sensitive populations.
Limitations
Much of the epidemiological evidence is observational rather than from controlled trials, and high-quality long-term data specifically on sensitive subgroups remains limited. Local reactions (like oral lichenoid lesions) are well-documented but are a separate question from systemic health effects.
What this means for patients
If you have intact amalgam fillings with no symptoms, major health authorities do not recommend removing them solely over mercury concerns. If you'd prefer a mercury-free approach going forward — for new cavities, or out of personal preference — that's a reasonable, evidence-compatible choice; composite and other alternatives are well-established.
