Researchers Identify a Molecular Cause of Dry Mouth and Gum Disease in Down Syndrome

A new study has pinpointed a defect in calcium signaling within salivary gland cells as a likely driver of the dry mouth and high rates of gum disease that disproportionately affect people with Down syndrome.

People with Down syndrome experience significantly higher rates of gum disease than the general population, but the biological reasons behind this have not been well understood. A study published in the journal Cell Reports by researchers at NYU College of Dentistry now offers an important clue: a disruption in how salivary gland cells handle calcium appears to be at the root of the problem.

The research team found that a cellular process called store-operated calcium entry — a key pathway that normally tells salivary glands to release saliva — is impaired in Down syndrome. When this signaling breaks down, saliva production drops. Because saliva plays a central role in rinsing away bacteria, neutralizing acids, and keeping gum tissue healthy, chronically low saliva output creates conditions where gum disease can take hold more easily.

The researchers also found elevated levels of succinate — a metabolic byproduct associated with inflammation — in the blood of their animal models, alongside shifts in the balance of bacteria both in the mouth and the gut. Separately, the team detected certain immune markers linked to Sjögren's disease, an autoimmune condition also characterized by low saliva production, suggesting people with Down syndrome may face an overlapping risk. The study's senior author noted that understanding and eventually restoring normal salivation in this population could have a meaningful impact on both their oral and overall health.

For patients and families, the findings underscore that dry mouth is not simply a nuisance — it is a biological condition with measurable downstream effects on the gums and beyond. Researchers say the next step is exploring whether therapies that target the calcium signaling pathway could one day be used to improve saliva flow and reduce gum disease risk in people with Down syndrome.
