A New Toothpaste Targets Gum Disease Without Disrupting the Oral Microbiome

German researchers have developed a toothpaste that selectively blocks the bacteria behind periodontitis while leaving the rest of the mouth's microbial community intact.

Most toothpastes and antiseptic rinses work by reducing the overall number of bacteria in the mouth. That broad approach does temporarily cut down on harmful microbes, but it also removes beneficial ones.

Conventional products such as alcohol-based mouthwashes and chlorhexidine wipe out helpful bacteria along with harmful ones, and when the oral microbiome regrows, pathogenic species such as Porphyromonas gingivalis rebound fastest — because they thrive in inflamed gum tissue — while beneficial bacteria lag behind, leaving the mouth in a recurring state of imbalance.

Researchers at the Halle branch of the Fraunhofer Institute for Cell Therapy and Immunology in Germany have taken a different approach.

Rather than eradicating microbes outright, their method works by inhibiting pathogenic activity. The active compound selectively blocks the growth of harmful bacteria such as Porphyromonas gingivalis, preventing them from exerting toxic effects while allowing beneficial species to repopulate and restore microbial balance.

Developers also had to ensure the compound was non-toxic, not systemically absorbed, and did not cause tooth discoloration — key barriers to clinical adoption.

The technology has been commercialized through the spin-off company PerioTrap Pharmaceuticals GmbH, which has produced a microbiome-friendly toothpaste and a professional care gel designed for use following dental cleanings. The formulations also include standard preventive components such as fluoride and abrasives.

For use in dental practices, a care gel is applied after professional dental cleaning, where it blocks pathogenic bacteria, stabilizes the oral microbiome, and keeps the gums supple.

The broader significance of gum disease prevention is not limited to the mouth.

The oral microbiome is home to more than 700 different bacterial species, of which only a small fraction cause periodontitis. These bacteria adhere to dental plaque along the gum line, triggering inflammation — and if they enter the bloodstream, they have been associated with conditions including diabetes, cardiovascular disease, rheumatic disease, and chronic inflammatory bowel disease.

For patients and practitioners interested in the connection between oral and whole-body health, this targeted approach to gum care reflects a broader shift in oral health science toward protecting, rather than simply resetting, the microbial environment of the mouth.
